The Role

The role of a Co-Care Partner is very hands-on, where flexibility is key. Whilst a large part of this role is about working directly with clients and helping in administering our clients get onboarded, their day-to-day operations, including aiding in the budgeting of their support plan, you will also have a strong focus on helping shape the direction of the company. You are someone that has great administration skills, attention to detail, a background in supporting older Australians, and basic knowledge of how the My Aged Care Home Care Packages work.

The role will look a bit like this:

  • Administering the day-to-day running of clients, from onboarding to managing the monthly check-ins and governance.
  • Assessing/processing invoices received on behalf of clients and ensuring that services provided/requested align with the client's support plan and government guidelines.
  • Resolving client and provider enquiries via phone and email
  • Building relationships with clients and other providers in the sector
  • Providing feedback to and preparing material to support PlanCare clients

This role is full-time, ongoing, and based in PlanCare’s Perth office.

We’re looking for someone who:

  • Is reliable and punctual
  • Has experience with the My Aged Care and Homecare Packages (especially self-management of HCP) and is confident with answering client questions related to their support plan and funding
  • Has superb communication skills, both written and verbal
  • Has a person-centred attitude and is committed to supporting people over 65 to live at home and have more choice and control over the support they need.
  • Knowledge and experience in case management.
  • Is able to create and maintain relationships with clients and other providers
  • Has a positive can-do attitude and isn’t afraid to share ideas and feedback to help us grow
  • Is a fast learner and can learn independently
  • Enjoys working in a team environment and having a laugh at work.

You’ll definitely need to have:

  • Excellent computer skills and attention to detail
  • Excellent problem-solving skills
  • A recent (less than three months) police check, or the ability to get one
  • The right to work and live in Australia

It would be especially great if you had:

  • Preference will be given to those who hold a current and valid Registered Nurse or Allied Health qualification
  • Experience in My Aged Care / Community Care
  • A qualification in Aged Care / Community care
  • Lived or learned experience of Aged Care
  • Experience using PRODA / MAC Portal / Services Australia portal
